A six-year-old boy has died following a three-vehicle collision on Highway 15 south of Smiths Falls on May 29, provincial police said Tuesday.

It marks the second fatality from the head-on collision involving two cars and an ambulance, which took place Friday at approximately 5 p.m. near Otter Lake Park. A 34-year-old driver from Elizabethtown-Kitley was pronounced dead at the scene.

The boy was travelling in the same vehicle as the 34-year-old who died at the scene, OPP said, and the child was airlifted to CHEO after the collision.

The driver and passenger of the ambulance were also taken to hospital with serious injuries and were released on Saturday. The ambulance was returning to its base from a call and had no patients onboard, police said.

The driver and sole occupant of the third vehicle was transported to hospital with minor injuries.

The crash site is about 90 kilometres southwest of Ottawa.

Police said the investigation remains ongoing. Anyone with information or dashcam footage of the incident is asked to contact Leeds County OPP at 1-888-310-1122 or anonymously through Crime Stoppers at 1-800-222-8477 or online at ontariocrimestoppers.ca.
